Work instruction for dyslexia care

Application

  1. The school builds a reading file, based on the Reading Problems and Dyslexia Protocol.
  2. The school and the parents sign the form 'Declaration of application for reimbursement of dyslexia care'.
  3. Parents are responsible for submitting the registration form to the municipality. The application can be submitted at the municipal office, but may also be sent by mail or email.
  4. Parents register with the healthcare provider for the provision of reimbursed dyslexia care, preferably via the provider's digital registration system.
  5. The school transfers (a copy) of the reading record to the provider.
  6. The municipality processes the notification in its system.
  7. The municipality ensures that provider receives an order (JW301 message).
  8. The municipality sends the decision to the parents.
  9. After receiving the assignment and the reading file, the provider will contact the parents to make an appointment.
