Work instruction for dyslexia care

Application

  1. The school builds a reading file, based on the Reading Problems and Dyslexia Protocol.
  2. The school and parents sign the "Declaration Request EED" form. This certifies that the school has complied with Step 1.
  3. Parents are responsible for delivering the application form to the municipality. The application can be dropped off at the town hall, but may also be sent via mail or e-mail.
  4. Parents register with the health care provider for the provision of reimbursed dyslexia care, preferably through the respective provider's digital registration system.
  5. The school transfers (a copy) of the reading record to the provider.
  6. The municipality processes the notification in its system.
  7. The municipality ensures that provider receives an order (JW301 message).
  8. Municipality sends decision to resident.
  9. Upon receipt of the assignment and reading file, provider will contact parents to schedule an appointment.
