Work instruction for dyslexia care
Application
- The school builds a reading file, based on the Reading Problems and Dyslexia Protocol.
- The school and parents sign the "Declaration Request EED" form. This certifies that the school has complied with Step 1.
- Parents are responsible for delivering the application form to the municipality. The application can be dropped off at the town hall, but may also be sent via mail or e-mail.
- Parents register with the health care provider for the provision of reimbursed dyslexia care, preferably through the respective provider's digital registration system.
- The school transfers (a copy) of the reading record to the provider.
- The municipality processes the notification in its system.
- The municipality ensures that provider receives an order (JW301 message).
- Municipality sends decision to resident.
- Upon receipt of the assignment and reading file, provider will contact parents to schedule an appointment.
